Register to access the ComChart Physician Patient Web Interface

Patients who have previously been seen in our office or have a scheduled appointment can:

review sections of their chart,

schedule an appointment,

request prescription refills,

file out an on-line history form

and send messages to their physicians via the web.

If you would like to signup for online access, please send an e-mail to checkin@ithacamed.com.

Once we receive your email we will send your username and password to the physical mailing address we have on file for you.
